Job Summary: 

The Home Support Worker (HSW) provides support and homemaking services to clients and families within the framework of VON policies, procedures, standards. The HSW collaborates with members of the health care team in meeting the needs of clients to attain an optimal level of independence and well-being. Home Support Workers do not perform client-specific delegated tasks.

Key Responsibilities:

• Performs tasks reflecting the needs of the client and their household’s safety, which may include personal care, light cleaning, preparation of nutritious meals, laundry, and  shopping. Observes and listens to the client and caregiver to identify whether the family’s needs have been met
• Contributes to the development of the personal support care plan by reporting any observed changes or needs to the Manager or Team Lead
• Implements approved changes to the care plan under the guidance of the Manager/Team Lead
• Provides respite care to clients as required
• Functions and reports in accordance with VON's policies, procedures, practices and guidelines designed to promote and protect client safety
• Other duties as assigned

Education, Designations and Experience:

• High school diploma or GED
• Certificate in Home Support Worker (Level I, II, III) is an asset
• Standard First Aid Certificate or CPR level C Certificate may be required
• Three years’ demonstrated experience providing support services within community healthcare
• Household management experience
• Experience working with elderly or ill individuals and/or children

• Strong verbal and written communication skills in a manner that demonstrates respect, empathy and understanding to clients, families and colleagues
• Good assessment skills
• Awareness of community resources
• Ability to work effectively in both a multidisciplinary team and independently
• Ability to take direction well
• Ability to work in various social and economic environments
• The capacity to adhere to strict confidentially

Other:

• Must have access to a personal vehicle for work and possess both a current driver’s license and proof of vehicle insurance for business purposes
• A current and original copy of a satisfactory Criminal Records Check
• A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required
• Must be able to wear Personal Protective Equipment (PPE)

Work Conditions and Physical Capabilities: Attention to detail; Lift and carry using appropriate lifting techniques; Bend, push, pull, grip, reach, kneel, crouch using good body mechanics; Walk, sit, stand, climb stairs; Hand movements.
