Care will take place within a residential or institutional setting with referrals generated by a variety of sources including government agencies, employers, insurance companies or private payees. <div><div>Job Description </div>What you’ll be doing<ul><li>Participates in resident admission process including tours, admission, transfers, discharge, and occupancy tracking.</li><li>Acts as liaison with families and advocate for residents to ensure they are allowed and encouraged to exercise their rights</li><li>Participates in the interdisciplinary team process of resident assessment, care planning and evaluation to ensure services are planned according to individual resident and family needs.</li><li>Provides therapeutic services through individual and group counseling for residents and families, documents on resident’s record and evaluates effectiveness of these programs.</li><li>Coordinates services of Psychogeriatric Consultant in support of staff caring for residents.</li><li>Adheres to established department plans, policies, procedures and quality assurance program to meet all regulatory and Extendicare standards.</li><li>Provides in-service programs for home staff, residents, and families. Encourages and supports professional growth and development of staff.</li><li>Liaises with educational institutions and provides supervision to social service work students during field practicum.
